Abstract : We present an unusual case of granulocytic sarcoma. A 75-year-old male patient presented with a painful enlarged uvula. A past history of mild thrombocytopenia was obtained. At the time of admission, however, routine peripheral blood film showed absolute monocytosis. A biopsy of the uvula showed a diffuse intermediate and large cleaved cell infiltrate, suggestive of a lymphoma. The initial results indicated no expression of either pan-B (CD20, CD79a), or pan-T (CD3, CD45RO) markers. Subsequently, antibodies against CD43, CD5, CD23, cyclin D1, CD45, CD68, CD56, CD57, TIA1, myeloperoxidase, muramidase and low molecular weight keratin were used. The neoplastic cells were negative for all markers except for CD45, CD43, CD56, CD68 and muramidase. A Leder’s stain was negative. A diagnosis of granulocytic sarcoma was established on the basis of expression of the myeloid markers and CD43. A subsequent bone marrow aspiration and biopsy showed the presence of chronic myelomonocytic leukemia in transformation (CMML-T). The expression of CD56 has been associated with myeloid leukemia frequently associated with extramedullary involvement. This is the first reported case of granulocytic sarcoma of the uvula.
